Enrollment

An application for enrollment at Alamance CDC must be completed by a parent or guardian with complete information given on the form, and a written statement of any special needs your child/children require - i.e. allergies, medications, unusual habits, etc.

A completed application does not guarantee enrollment.  If space is available, a visit will be scheduled with both the parent(s) and child.
